Do’s
for taking Application & Registration for
“A” and "B" Category
|
- Make
a DD of Rs. 1000/- for A-Category and Rs.2500/-
for B-Category towards application &
registration fees from any Bank in the favour
of “Dr.
VRK Womens' Medical College”
|
- Carry the Original EAMCET-12 H.T and Rank Card, Intermediate Pass Memo, Latest T.C. and show it at the Counter. Submit the DD and One Set of Xerox of all Certificates to obtain the Application Form.

- Fill the Application Form legibly and submit with a copy of the following Self-Attested certificates.

Copy of EAMCET-12 Hall Ticket & Rank Card |
ii. |
Copy of Intermediate/ 12th Exam Memo |
iii. |
Copy of SSC/ Equivalent Exam Memo |
iv. |
Latest TC/ Migration Certificate |
v. |
Copy of the Passport/Driving License / PAN Card/Voter ID as proof of residence of Parents. |
vi. |
Bonafide & Character Certificate from the last Institution to be enclosed, which would mention the status of his/her behavioral pattern specially in terms as to whether he/she has displayed persistent violent or aggressive behavior or any desire to harm others. (This is required as per letter No. MCI-34 (1)/2009-Med/5191 to 5401 dtd. 30-4-2009). If in case, this is not available at the time of registration, then it must be submitted at
the time of admission |

- On
submission of Filled-in application form along with
all the enclosures, Registration Slip must be
collected.
No separate registration charges
are to be paid.

- If you are submitting the downloaded application form in person then the form number will be issued along with the registration slip.

If submitting the application form (original/downloaded from website) by email, the filled-in form and enclosures shall be scanned and attached in zip format. The name of the Zipped file shall be EAMCET HT.No: for A-Category and Name of candidate for B-Category. The scanned forms should be send only to drvrkwmc12a@gmail.com for A-category and drvrkwmc2012b@gmail.com for B-category. Application forms sent emails other than above will not be accepted.

In case if there is any difficulty in transfering application fees online, then make a DD and send its scanned copy along with the application form. The original DD shall be submitted before counselling.

- If you are sending the downloaded form/ Original Form by post make sure it shall be delivered before cutoff time.

- No
application will be accepted either
in-person/by post/ or email after the cut off
time.
